Keep your health details private
Please do not send personal health information through any web form. That means no symptoms, diagnoses or medication lists in the form. Share those by phone or at your visit.
If you need to cancel or change an appointment, FCCI asks for at least 24 hours notice.
Emergency? The booking line is not for emergencies. For chest pain, fainting, trouble breathing, or stroke signs like face drooping, arm weakness or slurred speech, call 911.
Have a question about an existing appointment? Call the office during business hours and the team will help.
Your privacy matters, and keeping health details out of forms is one simple way to protect it.
Thank you for helping keep your information safe.

What to bring to your first visit
- Photo ID and insurance card
- A list of all medicines, including blood thinners and supplements
- Readings from a home blood pressure cuff or smartwatch, if you have them
- Notes on when your symptoms happen and how long they last
- Any records from past heart tests or hospital visits
Bringing a family member or friend can help you remember what the doctor says.
Arriving a few minutes early gives you time for paperwork. If you have questions after the visit, call the office during business hours.
If you have a heart device, bring its ID card. If you use a home blood pressure cuff, you can bring that too.
A short list of questions helps too.
